I know heat shields rattle like a *********** not saying it is that. It would drive me crazy too especially when it doesn't always do it. I'm sure you looked but fan blades skimming anything, loose tape flapping around. I did mention there is some kind of heat shield near the driver manifold that fell off of mine. Could'nt even see where it came from its so tight in there. I'm telling ya, go get a cheap harbor freight $6 stethoscope. You'd be surprised how that thing narrows it down. I know sound can travel through vibration and throw off its original source. Do you at least have it narrowed down to a specific area? Maybe it will get more consistant if that's a good thing

My 2006 does this always now on start up after being shut down for a while. It goes away after say 2-3 minutes. I'm thinking chain tensioners bleeding down but sometimes I'm thinking exhaust manifold leak. I wish there was a way to build oil pressure from an external source and then start the cold engine to see if it clatters.

If you hold the gas pedal to the floor and turn the key the engine will not start but it will build oil pressure. Then you can start it and see if the rattle is still there.

It doesn't build up enough oil pressure doing this to matter.

To the OP, my 05 has done this the last two years. Just depends on how she's feeling that morning I suppose:

No guide bits in pan, Napa Gold oil filter, Synblend oil and otherwise generally maintained as it should be. Rattles the one morning but doesn't the next.

Well just some morning cold? How many miles? And probably just oil pressure the oil on cold day not making it as fast and just knocking till it lubes everything. And if its doing it any weather and through out drive op low? If no are you useing right oil 10w 30 does it do it towards end of oil cycle synthetic isnt all great 6000 miles to change but trust me it breaks down before. Depending on temp of area and your op try less w oil or thicker and add bottle of Lucas oil instead and change 3000 probably will stop it. Check timimg chain they dont always sound same. Some get load especially 4.0 exp. But most likely you at 225 hauling loads daily so its running nice hot and heavy. So get rid of synthetic to thin. Get little thicker oil with lucas and change oftan get good filter to help from breaking down. Prime it before cold starts. Cause hauling makes difference on break down and synthetic really isnt all that. Go back to 3000 and little thicker to help coat everything.

No loss in performance?
I haven't witnessed any loss of performance in mines...yet.

I just pulled a 12,000 lb trailer for the church the other day.

And yes, it's almost like she's grumpy one morning or evening, and motivated the next.

Hot and Cold startups. Sometimes she'll do it, sometimes she wont.

I've been changing my oil every 3,000 for the past 4 years now. I pull trailers and haul loads. So that's why I change every 3,000 miles.

About to reach 229,000 miles soon.

Every figure this one out? Did you rule out the heat shield? I just started hearing something any time it is idling. I haven't been hearing anything when driving though. Everything else sound similar though...
