I have a 2005 screw 5.4 that was i was going to a gotts mod and SIDO exhaust. I keep reading about these motors running lean. My questions are what do you and how do you hook up to find out if it is lean. Will a good scan tool with live streaming work or do I have to get a custom tuner?

You will need a good scan tool that can read short and long term fuel trims. If you are going to spend the money might as well get the tuner. Although you should be fine with the gotts mod. It's the CAI's that change the MAF location or use larger intake tubes that cause lean issues.
