5.4 engine issue

5.4 engine issue
So my 05 with 5.4 in it has recently started idling erratically when I slow down. It doesn't happen often but it will idle badly and has even died a few times.
I can put it in neutral and it will start back up with a slight puff of smoke.
It has also had a drop in MPG it gets about 10 at the moment. The motor has less than 10K miles on it and was replaced at a ford dealership. They did a block swap but i think all the accessories were swapped over. This issues happened about 3 months ago and i changed the throttle body and it seemed to fix it until now. It has no codes and this is the PID from the scanner:
