Looks like 1. It for sure isn't 3, and that is the only one that doesn't have issues.

I'm not having any issues with my truck's TPS, but I figure I might as well update the TPS with a gen 3 one.

The wrench light is usually because of the TPS. My truck would run fine, and then at completely random times, would drop to 300 to 500 RPM's at stop lights. I ordered the part on Amazon, installed it and haven't had any issues since.

Would a failing TPS cause a condition where there's a slight delay in engine response when pressing the gas pedal or does it just cause a shutdown?

My tenant's '05 5.4 will do this delay when hitting the gas to accelerate when driving, whereas my '06 is instant throttle.

I was initially thinking fuel injectors - dirty or need replacing.

TB/MAF has been cleaned, FPDM replaced, new plugs/boots/springs.....

From my basic understanding yes. Rough idle. Hard starts. Poor acceleration, among other things.

has this become (or has it always been) a maintenance item???...I say this because my 2007 has some of these issues on occasion but my engine suffers from some of the other issues our 5.4 3V engine has and I'm unsure if this is a worthwhile replacement item...I would think the worst that I might experience is no change other than a lighter wallet...I do not get the wrench light

Mine exhibited surging sometimes with only slight foot pressure on the pedal. I replaced the entire TB (comes with new TPS), replacing just the TPS made the conditions worse, presumably because 2 old sensors could communicate better than one new one and one old one. After that, problem solved.

looked at mine today and it appears to be the newer, square style...perhaps the 2nd style as it is square on top and rounded near the bottom...
**added**I have the 2nd Gen TPS**

Of course I completed the TB relearn sequence, if that's what you're referring to.
