1st gear hesitation/stall at full throttle

Same problem
Hey guys,
Got the same problem on my '06 XLT 4x4, 5.4L.
My symptoms are a missing / bucking at around 1000-1250RPM when in top gear under a light load (starting up a hill)... have to feather the gas to get it to downshift....
Also, if I get on the throttle and try to bring the revs up above around 2800RPM, it will miss and hesitate from about 2800 to around 3250RPM. It will happen if I start from a stop, or kick it down to pass/accelerate. The harder I press the throttle, the worse it gets.
This has been happening since around 50,000K at 2800 to 3250, and started the 1000-1250 problem around 60,000K (or at least, that's when I noticed).
Some times if I try to hold the throttle down (not to floor, 1/2 to 3/4) when it starts missing at ~3000RPM, it will miss so bad that the Check Engine will start to flash... and even if you come to a complete stop, and try to drive normal, it will miss all over the place, and be undrivable. To cure it, I can simply cycle the ignition, Check Engine will go back out, and it will act normal again.
Unfortunatly, I've never had a scan tool hooked up when it did this...
I'm up to just over 140,000K (~84000 miles), and it's still the same as at around 60,000K, except the problem at ~1000 RPM seems to have gotten a bit worse.
Plugs have never been done, but since it has been happening since ~50,000 I don't believe they are fully to blame... and the truck runs really good otherwise... LOTS of power, smooth... smooth at idle...
I have been suspecting vacume as part of the issue, as I've also notice the odd time that I hit the brakes, or accel hard, my vents will shift to panel briefly, and/or my IWE's may try to engage (ratcheting sound, or worse... a big clunk when the axle engages the hub).... making me think I'm losing vacume.... going to try checking that in a few minutes.
Hope someone has found the cause.... I'll report back if I find anything going on with my vacume lines...
Oh yeah, I forgot to mention... fully stock... no intake mods, cold air, exhaust, nothing... bone stock
