So i got my new FPDM yesterday and this is what i found when i lowered my spare. I also had a seized lock that covered the spare tire crank tube .

Have you cleaned your throttle body yet? It's easy, super cheap and needs to be done periodically anyway.

Upon thinking about it for a second, you might want to go ahead and replace the front O2's also.

ok so i have noticed that the only time my engine bogs down is at around 1200 RPMs and i start to slightly accelerate. then if i let off the gas and then press on the gas again its fine. does this help to diagnosis?

5 years later if your still on this forum. Did you ever fix the problem for good?? I’m curious cause I’ve been chasing the same issue.

I'm having this exact same problem on my '04 FX4 5.4. Anyone ever find the true fix?

Default It seems the answer is different for almost everyone.

I tracked mine down and got ride of the code toward the end of July 2019. I had rewired the plug for that oxygen sensor because it had slipped down and started melting on the manifold. The code persisted after I rewired the plug so I replaced the sensor. Code persisted so I tried a billion other different things (checking for vacuum leaks, broken or melted hoses, ignition coils, fuel system, etc.) to no avail. As a last resort shot I decided to replace the sensor again with a genuine Bosch one. My first replacement was some brand I haven’t heard of before. My mistake... Haha but the Bosch one with the rewired pigtail did the trick. Haven’t had any similar issues since. Best of luck to ya. Most likely a sensor, wiring, or pcm problem with the P2195 code. Not many things should cause that specific of a code without any others being present, if I’m making any sense.
