Not sure what phone you have, but normally if you answer the phone with the physical phone, you should go in privacy mode. I know the iPhone does this, also my cheap motorola work phone does the same thing.

Just make a hotkey on your phone to turn BT on/off. Two presses of a button and can toggle it on/off. I do this on both my work and personal phone so I can connect sync either one to the truck very quickly. Both phones are 2 years old or older so I'm sure vast majority of them have a way to add a shortcut to the front keys. I can disconnect one and have the other one connected in about 5 seconds.

Mine will just turn itself off automatically. Especially when it knows I want to make or receive a call.
