I've had the same problem with mine twice. Once on me and once on my wife, very
aggravating when you can't fill it up. You then look like an *** when you can't put gas in your own truck.. From what I've told, it has something to do with the angle of the nozzle. Some are not angled enough to hit the tabs in the neck to open the flap.

I bought my 2011 pre owned from a Ford Dealer, it was very low miles but obviously filled up a few times. Still during the first month I owned it it seemed to be way more difficult to get the gas nozzle inserted into the capless contraption. It would always go but sometimes on the second or third try.
Now, after owning this truck for almost 2 years, I think the dang thing is finally "broken in" because I never have a problem, just slides right in every time.

The fuel nozzle has to push in on two tabs on either side of the cap less fuel inlet. If for any reason the station has the wrong size nozzle on thier pump it will not allow it to open up your fill cap. There are different sizes for different fuels.
The station has either rigged up the wrong nozzle to save money on purchasing the correct one, or on some nozzles they have the small nozzle with an adapter made to it to convert it to gasoline specs. It's very possible the adapter had come off and there fore wouldn't open you cap.
