Keepin' the lights on!
I really like these ones:

Welded onto bed length rails with a 1"x2" tie-down rail on top like this:
2nd choice would be a "Western Hauler" type something like:

B&W makes it. Same co. that makes the turnover ball gooseneck. I've done a few that I've welded a receiver hitch under the top tubing in the center or a small winch plate. Makes it nice for getting large game in the bed.

My list, who makes those side rails?

Talk to Aaron.
But you could probably have any good fab shop make you some. They usually have those rails bent for them. Meaning they start out with a 6" or 8" x.25" flat strap and have an inside rail (about 1.5") broke. They could also use some 4" or 6" x .50" flat strap and forgo the inside rail. For the top rail 1" x 2" x.125" rec tubing.
EDIT: Forgot to add that they cut the radius of the truck bed into the top rail (not the rec tubing) as well. They have templates for just about everything. Although, they aren't the quickest at doing what they do.

Wow. They are generally 350-500. Are you sure you didn't get an entire flat bed with the headache rack. The company I used to work for was a dealer for protech. My work truck had one and it is an awesome piece. Protech is the way to go with headache racks and flat beds. Weatherguard is the way to go with boxes.

Nope it was probably about 4 years ago when they were pretty new. But now I really like the magnum headache rack.
And this was a homemade one from a guy on this forum and I think it looks great.
