Sounds to me that somewhere between where you patched the hole and the end of your exhaust you have a stoppage in the pipe. Was the hole before the muffler?

5.4's of that age usually have an issue with a small rubber hose that crack/degrade and can collapse. I believe it runs into the throttle body, have to access it between the firewall and the intake. Worth checking that out while you are troubleshooting.

it was behinde the muffler. i switch from a single to a duel about 7 years ago. the quick fix i made and it worked for about a week or 2. then i left for work. the truck ran fine for about a mile or so then when i made a uturn i hit a good sized bump that jurked the truck a great deal. after that i hit the gas pretty hard and thats when it started to happen. now when i start it it runs fine untill it gets to indle and then the truck runs ruff.

Happened to me once when my positive battery connection came loose, or when my battery was going out. Not sure why the alternator didn't take over...
Or maybe a fuel deal.

When's the last time you replaced your fuel filter? I changed mine and no more bucking in higher gear with lower rpm

Sounds like a vacuum leak like cincyfx4 said. Especially if she won't idle. Double check all your rubber hoses, and listen for vac leaks. It could be your idle air control motor is gummed up, worth cleaning if you don't find bad hoses.
