1) yes, he should have used a treesaver.
2) yes, regear both diffs
3) yes..... he should have use a snatchblock.
4) idiot friends being way too close.
if i was in this situation, surely i'd have it in neutral.... and stand way the bleep back while using a wireless remote.
but, as i used to work in moving drilling rigs... here's a pic of a winch you really pray never breaks...

This is called a MICLIC. Each of those white bags have a block of C4 in them. They are fired out to clear mines (I'm sure they have used them for all kinds of things.
However, while I haven't seen them in action, I have seen them being made
The rope is pulled by a winch to the point a full sized man can walk down it and it doesn't budge a millimeter. Then workers tie knots onto the rope to keep the blocks from sliding.
It takes ***** to stand next to that thing. I haven't heard any stories of the rope snaping but I have heard about the winch being sheared off the floor and flying through the room before busting a few walls down.

I have seen pictures of a tow hook snapping off while a truck was being winched on and the hook and cable came flying backwards and went through the windshield and then completely THROUGH the drivers seat. fortunately the driver was using a remote and was standing next to the truck.
when winching a vehicle I will even open the hood on the vehicle doing the winching just for that extra layer of protection. it wont do much but it may deflect the blow away from my head.

I fully believe you about that hook going through the drivers seat. As for the hood being up, if we were using a front mount winch we always put the hood up as well and left the drivers side door open using the remote and standing behind the door. We were told that if we were pulling a supper heavy load to duck under the doors window glass so that there was metal between us and whatever part may fly at us. I personally hooked to a Chevy Z71's tow hook once to pull him up out of a very steep ditch he slid on ice and off into and it ripped that tow hook right off. Fortunately i used a double weight bag on the cable and it just dropped to the ground.
I watched a video during training and seen some crazy stuff happen with winches cables and tow chains. I think the most amazing thing i saw in the video was a cable snapped back and went through both front and rear windows of a trouble call truck and just did hit hard enough to snap the valve off of an oxy torch tank and the tank was chained down but it still ripped through the bed of the truck and done some damage. Thankfully nobody was hurt then either!
I dunno the rules about religion here on this forum but i know a lot of us have got into sticky situations and there just had to be some higher being watching out for us on those days lol

When they get it all laid out, they blow it. It's pretty impressive. I've never gotten to see it in action except on video though.
