bearing repair kit (for upper bearing) with all replacement small parts (wedges, spring, nut, bearing etc) needed F6AZ-3605-AA. Kind of pricey though cost me $80. Dealer didn't even know this kit existed but I found it referenced on this site.
Lower Bearing 30mmx19mm F4DZ-3517-A
There is also a small plastic spacer that push fits the bearing into the big black plastic bearing mount but I didn't get the part number for it.
