Running at such a low rpm isnt helping, ~600rpm, even my GM 5.3 shakes and burps idling at ~500rpm, i turned up the idle rpms on it, all good, no tuner on f150 yet.

My truck does this as well. 2.7 2016xlt. I look down and never see the rom change. I disabled my start stop and that hasn't affected it. I also notice it without the AC on - I rarely even run the ac. I'm going to ask the dealer...

A bad engine block is not about "test and fix", it's about sloppy quality control, plain and simple.
It's the hallmark of American manufacturing and precisely WHY people buy Honda and Toyota.
Go research the Ridgeline. You won't find dozens of posts about blown engines, ceased transfer cases and warped drive shafts. Ditto Tundra and Taco. It's a GD shame!

Tundra and Taco are American built. You obviously haven't seen the horrible amount of issues are with the 16+ Tacoma. Y'all think the 2.7 is having an issue? Y'all ain't seen anything until you take a look at the issues with the new tacoma

Interesting.

One thing that on my truck I've noticed is once the transmission gets up to running temperature (via gauge) the idle roughness in drive (only with AC on) is greatly diminished to the point of it's hardly noticeable.

A post full of fail. How many Ridgelines has Honda sold? How many F150's has Ford sold? I would bet if we had numbers from the manufacturers on a % base that they would be very similar.

What happens if you shift into Neutral while at a prolonged stop?
Does the shake/shudder get better?

Spark plug gap will cause this too, at least on the 3.5 it does. Thats how I know it is time to pull the plugs, clean and regap them.

Yes, the roughness goes away in neutral or in park.

I have a mildly rough idle at times but nowhere near dying. There is a TSB on the throttle body for issues like this you might see if it is applicable.
