Truck is totally stock except wheels/tires.

EDIT: and the other issue is this is an intermittent issue (at least up until now)

I will report back tomorrow.

Ok cold start this morning sounded perfectly fine.
Intermittent-sounds-like-a-blown-motor syndrome.

Do you park your truck on level ground every night? Also how long does the rattle last? I can't remember, is it only on the passenger side of the engine?

Yeah it's level ground, and both sides of engine.
