Default 89 f250 5.0 MT, tranny issues

I have recently acquired a 89 f250 standard cab long bed 5 speed 4x4. It starts and runs fine however I think the tranny has some issues. First the tranny is louder than I am used to. It goes like butter into 1,2,3 then grinds into 4, and slides right into 5. What is weird is that while its in forth it quiets down like a normal tranny would sound. The sound is like a heavy rolling or whine that stays consistent with speed of the truck.

Any suggestions?

Thank you for your suggestion. I was thinking this could bad bearing. Hopefully the gears and the forks are savable. Any thoughts on how much this may cost me a shop? I have worked on lots of cars but I have never cracked open a transmission.

That is again for your suggestions
