Did you try the link in that PCV diagram's caption? It's Motorcraft EV49B, which seems to be D8TZ-6A666-B.

Got the grommet in today and it definitely made a difference in fit, nice and tight. As a test, I ran it with the pcv that is already there for about 10 miles. Still leaking from up higher though. Definitely not coming from the pcv or grommet. Checked the valve cover bolts too and they are all tight. I guess the valve cover gasket could be blown out but it was replaced with a Felpro like 6 months ago by a reputable mechanic. Any other ideas on what it might be?

Just had the mechanic take a look at it. He said there is a small leak at the header that basically burnt a hole in the valve cover gasket. Needs a new intake/exhaust and valve cover gasket. Probably around $300 or so. I know the parts aren't a lot but I just don't have the time to get into doing it myself. Taking it in a few weeks.

Your first pics show the stock exhaust manifolds - not headers.
