Wrecking yard for an entire replacement of the rear differential. The 3.55 ratio is most common ratio to be had and can be exchanged in an afternoon. Switching over to rear disk can be done with a kit that is offered but it's fairly pricey. Use a yard that will warranty the part and if it has problems just have them send you another one out.

Agree with sdmartin find yourself a complete rear end at a wrecking yard or even better on craigslist. They run $100-150 around here on CL, $250 at yards.
I wouldn't mess with rear disc brakes - the conversions are a bit of a PITA and the drum system on these trucks is effective and cheap.
