I had a '91 F150 with a 5.8 that I bought new. It had an E4OD.
As to the original question, if the problem does actually stem from a T/C failure, its polluted the entire transmission. Either way you're looking at an overhaul.

So a torque converter and transmission swap could solve all the problems?

Thing is, the E4OD is an electronically controlled transmission, shifting controlled by the ECM, which uses input from the PSOM - electronic speedo- which the 90's didn't have, they had the cable drive speedo.
I'm just thinking out loud here and maybe somebody can correct me, but it was my understanding that to put an electronic transmission into a cable drive speedo truck (lots of people have) you would have to use a stand alone computer to run it.
I may be wrong on all that but... if somebody swapped that trans in, the whole computer control thing could be what's causing your issues possibly.
Worth doing some research on before you spend money.

My 90 also has the E40D. OD button on dash.

I always assumed the same as you but some googling revealed 1989 as well.

Which raises lots more questions for me than provides answers - mostly because of the electronic/cable conundrum you talked about in your earlier post.
