Default '88 Custom Ignition Accessory Problem

Hello, and thank you all in advance for helping me with this issue.

I have a 1988 F-150 Custom 4WD (5.8L) that I just installed an aftermarket stereo (Sony MEXN5000BT) in. It seems the previous owner cut the pigtail connector off of the original stereo wiring harness, and after a bit of research online, I found that most people are saying:

Car Radio Battery Constant 12v+ Wire: Green/Yellow
Car Radio Accessory Switched 12v+ Wire: Yellow/Black
Car Radio Ground Wire: Red or Black
Car Radio Illumination Wire: Blue/Red
Car Stereo Dimmer Wire: N/A
Car Stereo Antenna Trigger Wire: N/A
Car Stereo Amp Trigger Wire: N/A
Car Stereo Amplifier Location: N/A
Car Audio Front Speakers Size: 6″
Car Audio Front Speakers Location: Doors
Left Front Speaker Positive Wire (+): Orange/Green
Left Front Speaker Negative Wire (-): Black/White
Right Front Speaker Positive Wire (+): White/Green
Right Front Speaker Negative Wire (-): Black/White
Car Audio Rear Speakers Size: 6″ x 9″ Speakers
Car Audio Rear Speakers Location: Rear Corners
Left Rear Speaker Positive Wire (+): Pink/Green
Left Rear Speaker Negative Wire (-): Pink/Blue
Right Rear Speaker Positive Wire (+): Pink/Blue
Right Rear Speaker Negative Wire (-): Green/Orange

I'm not at all concerned with the colors for any of the speaker wires, because I've run all new wires for my speakers. Also, I confirmed above to be true by testing with a multimeter.

My stereo is working fine, but I am having one problem. The Yellow/Black wire for the +12V ignition "switched" feature is giving a +12V reading when I have the key turned fully forward, and when I have the key turned all the way back (position for removal). I can completely remove the key from the ignition, and the stereo will stay on. There are two positions between fully back, and fully forward on my ignition. While I'm in either of these positions, the stereo is off, and the ignition is locked (I can't remove the key). My new stereo has a removable face, so to keep it from draining my battery, I just remove the face. I really hope someone out there can help me out with this issue. I would like to get an alarm sometime, and be able to leave the face on the stereo every once in a while.

You keep turning it back to accessory and the key is coming out when it isn't supposed to.

Are you sure? So basically you're saying that my ignition switch was installed incorrectly or possibly broken? Because the only time I can remove the key from the ignition is when it is in the fully back position. Why would the key not come out when it's one click forward from fully back? Is there a way I can prove your theory to be correct? EG. will any other accessories work when the ignition is fully back and/or the key is removed?

All the way back is accessory, so the lock cylinder may be defective. The wipers also work in accessory.
