So I changed the fuel filter and it's running a lot better. The shuddering is minimal but still there.

Did you check the fuel pressure?

You have to get a kit. I would recommend the one from harbor freight. Cheap and has everything you need. Then I would watch some videos on YouTube about it. Not too difficult. If you have any questions ask away

I'd be almost certain that it's a faulty coil that is causing this. I get the same issue with my 5.4L triton. Coils start to go, they cause a misfire that almost feels like a hesitation around 40mph. The longer you drive it without fixing the worst it gets until the coil fails completely and then you'll be running on 7 cylinders. It usually doesn't kick on the CEL until this happens. Change all 8 coils and move forward. I bet it fixes your problem.

Does this more when accelerating with minimal throttle. The more throttle the less the hesitation is felt during accel.

That's what I'm thinking but since my truck is a 93 I was going to change the cap and rotor and coil? What do you think? Plug wires too? Plugs are new.

The coil is extremely common to have issues on the 5.4 but much more rare on these trucks. I guess it's possible but I kind of doubt that as the issue

Check fuel pressure as was mentioned. Also test the TPS for a flat spot.
Look thru my posts for 'testing the TPS'. You only have to worry about the signal wire test for starters.
