I think my next step will be to change sensors. The truck has neary 350,000 Kms on her, so I'm willing to bet them sensors are worn. What sensor would affect my fuel mileage? The o2 sensor... Anything else?

It's a Canadian thing eh!
Getting kinda old, people complain about poor fuel mileage with the I-6. Then you ask em, what's you're mileage in numbers, or what do you consider bad? And they say maybe 14-16 mpg combined lol. That right there is the engine in almost its prime efficiency under normal conditions.
Sorry just makes me laugh whenever I hear that

Most after market sensors are not the same quality as the OEM. The only sensor I would change is the O2 sensor. It lives in a harsh enviroment and can get dirty when the engine runs too rich or is burning oil or coolant.
Back in the early days before EFI it was recommended that O2 sensors be replaced every 60k miles, and items like the EGR valve had a high failure rate if not cleaned.
Most of the other sensors will trip a code if there is a problem and can be tested with a cheap DMM (Digital Multi Meter) Most Map sensors, MAF sensors, and some knock sensors are frequency based, so a DMM won't help but they can be tested other ways.
My Nelly Belle gets an average of about 16.5 to 17.5 in mixed driving. Back when I had skinny stock 235/75 tires I could get up to 20-21 Mpg at a steady 55-60 Mph.
I did change out the stock spark wires for Taylor spirial core ones, bumped the timing to 12 deg, increased the gap to .050. This increased performance but mileage stayed the same. With 244k miles I have no complaints. I love the low end torque and reliability of the 300 and I will miss this truck when its gone.
On my 94 300 and 83 302 changing out the stock exhaust for a free flowing system did increase gas mileage since the old ones were restricted.
