Mine has the same tag on it.
I'm working my googlefu on the number...hope I can dig something up.

With regards to the quick connect that you said was bunged up on this tube...is the nipple still inside the connector? If so, I bet you could pull it out with a drill bit, ez out, needle nose pliers, or a coat hanger...

Thanks for the help guys. I have two options try to epoxy the nipple back on. Two buy new intake tube.

still looking....
this is the closest to making your own...

fitting- looks close, but with the nipple gone.. not sure how close it is to a fuel filter fitting...dorman does make other sizes in this style too....

and this other pic looks like it just goes to a rubber hose...
