This issue has to be related to programming. Every time I reflash the truck it runs smooth for 500-1000 miles and then the shuddering/surging return. If I install one of my custom tunes the problem shows its self much quicker and I always end up going back to stock. I noticed I can reset the KAM with my SCT. I'm going to try this and see what happens,

Mine started as the 55-60 mph shudder under load(up a hill), then started it during slow acceleration, Ford did the 2 previous "CAC" fixes and it changed the shudder from 35-50 mph(5/6 gears) and then started misfiring bad upon heavy acceleration, as well as shuddered up hills.
I've only had the new CAC for 2 days, but all of these seem to have gone away, truck feels stronger, no longer have the constant boost dumping sounds under acceleration that I had after Fords previous 2 fixes. Pretty sure one of the previous fixes was to detune trucks until the permanent fix came out...cause mine runs like a raped ape again.

So, I don't have a Eco, but detuning, as mentioned could be the issue, and it could be an intentional flux in the acceleration.
All the CAC problems seemed to be related to running long distances at low RPMs, without any real change, or variation, in the acceleration, which allowed the condensation to build up to unsafe levels.
It seems the way to fix this, until the CAC can be replaced, is to have the accelerator drop back and then push back up, to vary the airflow, clearing the condensation.
The good news is, from one of the posts, it looks like the new CAC fixes it.

Had a similar issue with mine.
Slight uphills in cruise,shudder and jerk a bit.
I could hear the turbos dumping too.
Dealer did a TSB 12-3-14
Replace turbo regulator and update PCM.
Problem is gone.
