You DID state you replaced an injector with one from a junkyard. It's possible that's partially plugged...not feeding enough fuel into the cylinder which can give the impression of a misfire.
Pull the spark plug in that cylinder and see if it's burnt more than normal.
Did you replace the spark plugs or install the old ones back after cleaning and inspecting them?
Could also be a plug wire or boot was damaged during teardown.
If it's not the EGR try checking you plugs, wires, boots and the junkyard injector.

Hertfordnc.....You will not see any OBDII codes set until the truck has between 50 and 100 miles. It takes that kind of driving for all the sensors to the ECM to reach readiness and the codes to be displayed.
The Torque Pro App works well with the bluetooth ELM units on this system. Torque Pro is a one time pay, but it is so much better than the free App. If you want all the information available, it's got to be the Pro version.
I agree with Brew....check the EGR system. Going from power to dogginess is a common EGR trait. It could also be a sensor giving you grief, and even the fuel pressure regulator. This where I would start without codes to follow.
How is the #4 spark plug looking? Brew asked how they looked earlier, but I didn't see your response.

Please note that the bluetooth ELM327's do not work with iPhones, as they do not allow generic bluetooth devices. There is a WiFi version though. Also Torque Pro is only an Android app. I think the Apple users use ForScan.

I forgot about the iPhone. I use Androids. My GF just gave me her iPhone 6 plus. I may have to try it.

will partially plug, from the bottom up.
Spray pattern might resort to be a delayed dribble.... if that.
I tried using an old Windstar injector once...denied.

Thanks for all the input. I've had to pause on fixing this truck for a day or so to do other things. But I'll fill in some blanks from this thread.
This truck is totally beat, only 160K miles but not well cared for. smokes at startup, lots of dents, interior is ugly etc. For the price of a new intake I figured I could beat some more miles out of it learn some stuff but I did not want this job to nickle and dime me past my level of interest. I have friend with a pristine Lincoln Navigator with a blown HG I'm thinking about salvaging so this project was sort of a practice run.
Yes, I did change the plugs. Silly not to with the intake out. I put in the cheapest ones they had.
The injectors got mixed up so I can't identify the one i replaced. But It looked pristine, better then the other seven and the donor had been wrecked so i don't think it's a suspect.
I went back and applied dielectric grease to all coils coils on the left side and that helped a bit. when i get some time later this weekend I'll do the other side.
I've had the OBD bluetooth gizmo in the truck for a year as i worked through diagnosing the original problem (no.4 misfire) I'm using it with a Galsaxy S4.
Typically, if the code was cleared by the app of disconnecting the battery it would come back within a few minutes of driving.
But yesterday the LEDs went out. I thing the bluetooth thing is dead.

Humm Modular ford engines dont like cheap plugs that alone could be a problem. Motorcraft plugs.. You didnt replace boots either on an old engine.. The coils dont last forever.. The cheap way of fixing your problem is having the engine scanned with and obd II scan tool; that has MODE 6 live missfire data. Shows missfires live AND history.. They talk earlier in the thread about the touque pro app thats the one or an expensive scan tool. Other wise Replace plugs with the right ones minimum boot replacement and if you do that I'd just replace all the coils. Cheap route to pin point the problem you need it scanned. it ran before so.. it should run now.

So it looks like the consensus here is that I have a failure of some sort, plug, coil, etc as opposed to an installation error?
My OBD scanner is probably not broken, looks like there is a common fuse problem I'll look into.
What is the recommended app ? I've been happy with Torque Pro but I think there is more information I could be getting.
