Do you have any codes? Sounds more like an electrical issue like plugs, COP's. Engines will often run well at idle or with no load on them then start missing when you put them in gear.

If it runs fine under load in reverse more than likely it not a engine problem.

It is real hard to tell what is wrong without listening to the truck, knowing the maintenance history on the truck, and inspecting it. It is not for certain, but the condition of your transmission fluid might give a clue. If it is dark red or black, this is not good. It is really not good if it is brown and smells like it is burnt. Have you kept up with maintenance like changing filters, PCV valve, cleaning the MAF sensor, change your spark plugs? Has anything happened to the truck recently to create the problem. Like doing a lot of off roading? When did the problem first start?

All of this could give you a better idea as to what the problem is.
