Do you know the model number on the muffler itself? Also, would it bolt up to a 2012 Supercrew 5.0?

I'll look to see if there is one on the muffler. The kit part number is 15588.

Yes, it will bolt up to the 5.0 truck. The hangers and locations are the same. I run a stock raptor catback on my screw 5.0.
