what can I do to get better mpg
I'm going to collage this next fall about 150 miles away from home and I'm wondering what mods I can do to my truck to get a little more mpg. I have a 04 screw with a iron cross front bumper and 35" tires (want to keep both) I am looking into a tuner and plan on doing the gotta mod this weekend what else should I do ?

35's + 6,000 lb rolling brick of a truck + heavy accessories + a heavy foot ( yer young ) + a crappy inefficient drivetrain ( ~30% losses) = yep, you guessed it, poor mileage.
If yer serious, sell it and buy an econobox until you graduate and can actually afford what you want. Because the very fact that you had to post this in the first place suggests to me that you cannot.
Otherwise, it's futile. Other than returning to a stock config (and saving a couple mpg's), yer SOL.
Gotts Mod? No savings. Tuner. for ~$400? That's a lotta gas, and Still no mileage improvement is guaranteed, or to be expected. You. Are. Wasting. Yer . Money.
Putting lipstick on a pig - It's still a pig. Have ye cake and eat it too? Nupe - not happening.
Sorry - that's reality. Not gonna sugarcoat it fer ye - it's adult time, lol.
Brief list:
Stock tire size
Lightest weight tires you can find - Reduce rotating & static mass
Tires inflated to max
Keep your foot out of the throttle
Don't race or speed
Use cruise on the hiway
Use high BTU-content, high quality fuels
Weight reduction - keep bed /cab empty
Lower the truck - (aeros)
Comprehensive vehicle maintenance
Plan your trips - avoid prolonged idling
Take public transit when possible
Ride a bicycle ( my approach )
... and much more

That's what I expected to hear. So I'm stuck with 10mpg then? No chance for improvement because that's what I was getting stock and I'm light on the pedal, my aunt and grandpa have the same truck and get 15-18

No. Google Top Tier fuels. Running anything over 87 in yer gen truck is just wasting money - the OEM stock PCM calibration is designed to run on 87 - cannot use the extra octane.
Do Gramps and Granny also have 35's?
What I did neglect to mention was, given the increase in rolling stock diameter, your speedo and odo are both off, as well as yer trans shift points. This will skew any mileage calcs unless you correct for it. A programmer will fix the data errors, but not necessarily improve mileage to the point that it pays for itself.

That's why I'm getting a programmer , my buddy that owns a shop figured out my mpgs for me with the bigger tires. Would changing the spark plugs help ? not sure when the last time they were changed was

Is that city driving or mixed? I get 15.5 mpg mixed with 135k. If you're only getting 10mpg you need to look elsewhere like plugs and such.

the truth of it all is you have a pickup!
did you buy a pickup because of the great gas mileage?
you have 35's on now, like stated that is a check off the mpg
tuners can help and sometimes not help with mpg
you need to find what i call the sweet spot mph when driving down the highway just because it says 75 does not mean you need to do 75.
i am not trying to be a jerk off but when buying a pickup the last thing that should be talked about is mpg it is a pickup modded or not
