Could it be a plugged cat? With the way it's getting red hot by the cat it sounds like the exhaust gases aren't flowing through properly. Does it move at all? Does the truck die when you let off the gas? Mileage on the truck?

Sounds like a clogged cat or restriction in the exhaust if the exhaust is getting red hot.

170 000 on motor yes it will move as long as I don't hit the gas hard and dies sometimes but not all the time could I just drill a hole and release the pressure if so how big of a hole and thanks for the reply

That is a bad cat. Go to a good muffler shop.
