Did this on my truck a few months ago. I did attempt to go in from behind through access hole but it was impossible to adjust that way.
Good luck

When you look at the backside of the brake backing plate, below the axle, you should see an oval shaped rubber plug. Pop it out, and shine a flashlight through the hole. You'll see a threaded fitting and what looks like a gear on one end. That is the adjustment star.
However, since it has probably been a really long time since the parking brake was maintenanced, it would be a good idea to simply remove the brake rotor so you can examine the brake shoes to see how much material is left. Also, remove the adjuster mechanism, clean it, and lubricate the threads with high temperature grease (I use disc brake grease), then reassemble and adjust to where the rotor just does slide over the brake pads without resistance.
