I have an 04 with a manual driver side rear view mirror. It's so loose that at higher speeds it moves to the point I can't see out of it. Any suggestions? Do I have to remove the door panel?

Unfortunately, as far as I know, a new mirror ....

You can take the door panel off, and tighten the bolts holding the mirror on. That might fix it.

It is folding (most probably)from the pivot point where you fold it in and out. Both my mirrors did that after some nasty people bent them the wrong way at parking lots as they walked by. One was completely lose other one so so ...

best bet would be to look for some 09+ manual mirrors as they look like power mirrors but wont move at highway speeds
