in the second pic it shows where you first start to pry the panel out, there should be a small slot for a screw driver there, the next few pics show how to remove the panel, after you pop a few clips out with a screw driver you should be able to finish popping it out by hand, after you get it off there are two holes for speakers, one is a 6x8 hole and the other is for tweeters up above, why the factory didnt put tweeters in there is a mystery to me... lol but makes life easier for aftermarket installs, finally to put the panel back on you simply line up the clips with the slots and push (watch out for side mirror adjustment toggle, it will prolly get in the way)

but for my question, how do you get the back panels off in an extended cab???

There's a small bolt at the bottom of the panel and a larger one connecting the handle to the inside of the door. You have to pry up on the power window switch panel to remove it and then you can't miss it. Do not try to pry off the grille part because it does not come off.
