Did the link I posted in #4 above not work for anyone? It works for me, but then it's on my Google Drive, so naturally I have access to it.
Anyway, I don't think it's the suspension bushings, because I never get this noise going over bumps, no matter how big they are or how fast or slow I am going. It only happens when shifting from drive to reverse and back and then only when either the service or parking brake is on. I almost though it was the axle rotating in the leaf spring U-clamps, but I don't see any movement there on the video. I also don't see any movement between the caliper mounting bracket and the axle.

The last link worked. That sounds like the parking brake man. I'm on my phone, so the sound quality may be a little off, but it definitely sounds like the brake. That would drive me nuts! I know you replaced everything, so I don't see how it would be though. Could the brake cable be pulling the backing plate askew somehow? The only other thing I can of down there would be the axle bearing, but I'm sure you would notice other symptoms.

Just replaced the axle seals not long ago. I lubed the bearings with moly paste. The noise happens whether the parking brake is on or not. If no brakes are on, I don't hear it. If either or both brakes are on, I do hear it. It has to be brake related, then, but everything is brand new and it made the same sound before replacing the brakes.

My truck makes this exact same sound. It bothers me, but I've been so busy that I haven't had a chance to spend about an hour trying to rectify it. It's suspension related. The bushings for the leaf springs, or the the slide pads between the lowest leaf need lubricating. I did my rear brakes last year and have less than 10K on them, and my parking brakes were adjusted during that time. The sound was there beforehand, just like you are experiencing. Lubricate those areas and the sound should cease.
To lubricate the slide pads, you're going to need a set of jack stands to rest the frame of the truck on, so that the axle will free-hang without the wheels touching the ground. This way, the leafs are fully de-stressed and open up a bit, so you can see and lubricate them.
