Brake rotor removal

Brake rotor removal
I have a 2004 4WD F150 that I picked up a couple years ago (a 9/03 build so the hubs are a 5 lug pattern and I had to buy 2003 rotors). I'm trying to do a complete brake job but can't get the front rotors off. They seem to be rusted solid to the hubs. I've soaked them with PB Blaster, used a rubber mallet and a gear puller; they won't budge. Am I doing something wrong? The rust is so bad that one of the brake lines to the caliper broke when I moved the caliper off of the rotor.
Should I completely remove the hub assembly with the rotors on and have them removed or replaced? Any help is sure appreciated.

trade the rubber mallet for a small sledge ... just had to beat my rear rotors off with only 66K miles on them.

For future reference. You likely have an '04 Heritage. The truck will be an 04 model year, but look like the 97-03's. You shouldn't have much issue buying '03 parts, but if you tell places 2004 model, make sure you tell them 2004 Heritage.
If I remember correctly, there should be some identifiers on the truck... I think the fender and tailgate emblems identify it.

x2 on sledge hammer. You're replacing them so don't be afraid of marring them. Had to do that with a recent front brake job on the Misses' Corolla. Anti-seize the new rotor contact surface.
