my local ford dealer bodyshop, he made the template from a stock fender that was damaged, cut off damaged area and saved the area where the stock flares were mounted.

Not exactly leveled. Bilstein 5100's all the way around with the front all the way up and 2" lift shackles in the rear. It left a little rake and levels out nice with my trailer attached. Running 275/70r18 Duratracs on stock wheels for now. I'm planning on adding 18" BMF Novakane's soon.
