Default Clutch Help

It would seem my clutch isn't fully disengaging. When I press the pedal to take a turn and run the brakes, my rear end locks up and begins to give me problems (has a automatic locker in the rear). When I take the same turn with the transmission in neutral the locker stays unlocked. Secondly, when I put the truck into gear, the rear locks up. If I start off by just letting the clutch out, I get to a point in the pedal travel where the truck begins moving forward but also starts to violently shake which doesn't go away until I take my foot off the pedal. I checked the fluid today to see if I needed more and it was full. This clutch has 32K miles on it, was replaced at 95K miles (lost the throwout bearing), hasn't pulled anything more than 2500 lbs, and I tend to just float the gears on the highway. My guess that the clutch is just warped to hell, any suggestions?
