03 f150 transmission issue?

hi I have an 03 f150 5.4l . it jerks at a low rpm when driving on an incline as soon as the transmission shifts down and it revs at 2000 or higher its fine and ideas?

is it in overdrive when this happens then shifts out of overdrive? If so it could be a misfire.
