Default slave cylinder

slave cylinder went out on 96 truck again . this will be third time in 2 1/2 years first 2 were after market ones the last one was a ford part .. the shop doesn't know why? any advice

What exactly is failing?

I suggest to check to ensure that the correct fluid is being used.

Since I'm guessing that this is an internal slave cylinder, any possibilities of alignment issues when installing? Either not following a prescribed installation sequence (not familiar with internal slaves, just speculating), or something got bent?
