Did you replace anything that is belt driven recently?

like 93 said could be bad pulley, misaligned pulley, bad tensioner or idle pulley.

Take the belt off and check each one, with belt on look for any deviations in the belts path and check for tightness (a good sign that the tensioner is working properly). When I replace a belt I usually do the idler pulley as well, doesn't need it that often but they are cheap and quick to switch.
