IAC (idle air controller) located next to where the air intake hoses connect to the top of the motor. My 93 Flareside had this problem. Rough idle, wouldnt start when warm, stalling at red lights. Find it, remove it and either clean or buy one for $80. I cleaned mine with brake parts cleaner. They get alot of carbon build up and will cause the 2 little valves inside to stick. They only open at certain speeds and idle, this made my CEL come on an off eratically

Good idea. The IAC will not present a code if it's having problems either. Remove it, clean it, reinstall it, test it....as per method on this link:

Sorry for digging one up on you guys... Just the first one I've seen with the exact same problems as mine. I've taken out, checked and cleaned IAC and it helped for about 5 miles. I've tried injecvtor cleaners, I think I've tried most everything. I am convinced it is something with airflow because spark is fine everywhere all new tune up, fuel pressure is good with new filter. I've heard these IAC's are famous for going out, but this one was failry clean when I took it out and my truck has 240K on it. Any chance it could be a relay or something? CEL is intermittent, and only code I've ever gotten was EGR. They should lead to each other right?

Ever fix it? Sorry to dig this one up. AGAIN. haha But i have to as it is not resolved yet.
Anyone look into oil problems or anything?

If Jam can not pull codes using an obdi reader his ecu is shot and wont go into diagnostic mode.
