check play in the rear end. i had a bad pinion bearing and it was causing vibrations, also have your tires balanced.

Heater,
Drop the drive shaft and manually check the U-Joints. Try to make them rotate by hand. I bet that you will find one that is seized.

I recently had to have the carrier bearing replaced, along with the ujoint, and the shop also checked the drive shaft to make sure it was balanced as well.

Did it start right after they replaced the carrier bearing or did it wait some. Just because the u joints were good when they checked does not mean that they lasted much longer. If you have an auto block the wheels and put the tranny in neutral to check the u joints, if there is any binding you will not find loose joints.

Thanks to everyone who has replied . I was just able to dig into it and found all the bolts that hold the trans tail shaft case to the tranny were all loose. Thanks

No it has never been the shop for tranny work or any thing else for that matter.
it looks like they just came loose . It has been my shop truck for 9 years with not much trouble.
